Access tax reporting forms, withholding elections and required tax notices related to your retirement benefits. Many forms are mailed automatically, and copies are available online after the mailing dates.

Log in to Benefits Access to view available tax forms. After logging in, go to Retirement → Accounts → Tax Forms.

Annual Tax Reporting Forms

Wespath mails annual IRS reporting forms to participants, including:

  • Form 1099-R for retirement distributions
  • Form 1099-MISC when applicable
  • Form W-2 for disability payments

Additional forms may be mailed based on residency or tax status, including Form 480.7C for Puerto Rico participants and Form 1042-S for certain non-U.S. income reporting.

Copies of mailed tax forms are typically available in Benefits Access after mailing dates. Participants who cannot access their tax forms online can contact Wespath at 1-800-851-2201 for an additional copy. Please allow two weeks for mail delivery.
