Contribution Election - UMPIP - formerly the Before-Tax and After-Tax Contributions Agreement Authorizes the plan sponsor/salary-paying unit to deduct a specified amount/percentage from the participant’s pay to be contributed to the United Methodist Personal Investment Plan (UMPIP) on a before-tax, Roth or after-tax basis (for contributions on or after January 1, 2016). Completed, signed form is returned to the plan sponsor/salary-paying unit. UMPIP
Contribution Election - Horizon - formerly the Before-tax Contributions Election for the Horizon 401(k) Plan Authorizes the plan sponsor/salary-paying unit to deduct a specified amount/percentage from the participant’s pay to be contributed to the Horizon 401(k) Plan on a before-tax or Roth basis (for contributions on or after January 1, 2016). Chaplains Grant Fund Application Allows eligible clergypersons who have been endorsed by the United Methodist Endorsing Agency as a chaplain or pastoral counselor to apply for a grant from the Chaplains Supplemental Pension Grant Fund. Multiple plans
Claim Form for Survivor Benefits Allows survivors to apply for death benefits. Multiple plans
Depository Agreement for Payments Authorizes Wespath to send benefit payments directly to a participant's financial institution. Also used to update banking information. Note: banking information can be updated online via Benefits Access. Multiple plans
Designation of Beneficiary for Retirement and Welfare Plans for Participants Designates the beneficiary(ies) eligible to receive retirement and welfare plan benefits in the event of a participant's death under the terms of the applicable plan(s). Designation of Beneficiary for Retirement and Welfare Plans for Surviving Spouse/Non-Spouse/Alternate Payee Accountholder Designates the beneficiary(ies) eligible to receive retirement and welfare plan benefits in the event of a surviving spouse, non-spouse or alternate payee's death under the terms of the applicable plan(s). Note: Beneficiary designations can be updated online via Benefits Access. Multiple plans
Election to Rescind the PDA Allows participants to rescind their Participant Directed Accounts (PDAs). Participants with PDAs may participate in the LifeStage Investment Management Service only if they rescind their PDAs. Multiple plans
403(b) Contribution Planner Input and Instructions Provides participant data to Wespath to determine annual contribution limits. Multiple plans
Hardship Loan Debit Agreement Authorizes electronic funds transfer for UMPIP loan repayments. UMPIP
