Beneficiary Updates Requested

Wespath Benefits and Investments (Wespath) sent a letter to all participants in Wespath-administered retirement plans and asked them to review their beneficiary information and provide updates, if necessary. It is important to ensure that beneficiary data is current, so that assets are distributed according to participants’ wishes and beneficiaries can be located in the event a participant dies.

If you have not yet responded to our mailing, we ask that you either:

You were contacted at the address you have on file with Wespath. If we do not receive a confirmation or an updated form, a final update request will be sent via certified mail. Please help us avoid the expense of this fourth mailing, which will conserve plan dollars.

If the Information is Correct

If the beneficiary designations and related information on your confirmation are correct, sign and return it to us via fax at 1-847-866-5195, scan the signed form and e-mail it to prcwebteam@wespath.org, or mail it in the envelope that was enclosed or to the address listed on the form. If the percentage column on the enclosed confirmation is blank, please submit a Designation of Beneficiary for Retirement Plans and Welfare Plans form indicating the share of benefits payable to each beneficiary. If this information is not received, any benefits payable upon your death will be distributed in equal shares.

Do not make any changes and/or additions to the confirmation—changes can only be made using the Designation of Beneficiary for Retirement Plans and Welfare Plans form. If you have misplaced the confirmation form and do not wish to change your beneficiaries, please contact us at 1-800-851-2201 for a copy of your confirmation.

To Add or Change Beneficiaries and/or Related Information

If you elect to add or change your beneficiary designations or related information, complete, sign and return the Designation of Beneficiary for Retirement Plans and Welfare Plans form to Wespath via fax at 1-847-866-5195, scan the signed form and e-mail it to prcwebteam@wespath.org, or mail it in the envelope that was enclosed or to the address listed on the form.

In the unlikely event that the beneficiaries shown on the confirmation do not match what you believe is your most recent beneficiary designation, please complete the Designation of Beneficiary for Retirement Plans and Welfare Plans form as directed above. If we do not receive a completed form, the beneficiaries shown on the confirmation will remain your beneficiaries of record until you submit a new form.

Keep a Copy for Your Records

Your signature on the confirmation will help prevent challenges to your designations upon your death. Be sure to keep a copy of your confirmation or your beneficiary designation form for your records.

Wespath’s receipt of a beneficiary designation form does not guarantee that you or your beneficiaries will be entitled to benefits. Eligibility for benefits is determined by benefit plan rules. If you checked the “All retirement plans” box in Part 3 of the beneficiary designation form, your confirmation will list several Wespath-administered plans, even though you may not currently—or ever—participate in all of them.
